Residential Care for the Elderly (RCFE) Physician's Report

All prospective residents seeking senior care placement must have their physician complete a Physician’s Report, which is provided to the care facility. This report includes a required TB test or chest X-ray taken within the last six months.

Additionally, the form must be signed by both the primary care physician (PCP) and the resident or their authorized representative to ensure proper documentation and compliance with facility requirements.
